Terms of Service

The agreement between you and Sylhet Ambulance Service.

By scheduling a trip, calling our dispatch line, or using sylhetambulanceservice.com, you ("you" or "customer") agree to these Terms of Service. Please read them carefully.

1. About us

Sylhet Ambulance Service is a Texas-registered non-emergency medical transportation (NEMT) company owned by Osvaldo Martinez, headquartered at 2046 Chapel Street, Houston, TX 77063.

2. Services provided

We provide scheduled non-emergency medical transportation including wheelchair, stretcher, bariatric, ambulatory, and recurring dialysis services. We are not an emergency medical service. For life-threatening emergencies, dial 911.

5. Payment

Insurance-covered trips are billed directly to the carrier. Any co-pay is due at time of service. Private-pay invoices are due within 30 days of the trip date. Late payments accrue interest at 1.5% per month (annual rate 18%) or the maximum permitted by Texas law.

6. Liability

We carry $1,500,000 in commercial auto liability insurance and $1,000,000 in professional liability. Our liability is limited to the amount of these policies. We are not liable for delays caused by weather, traffic, third-party medical facilities, or events outside our reasonable control.

7. Patient conduct

Patients and accompanying family members are expected to treat crew members with respect. We reserve the right to refuse or terminate service in cases of threats, intoxication, weapons, or verbal abuse.

8. Equipment and personal property

Patients are responsible for their own personal belongings, mobility aids, and medications during transport. We will assist with loading but are not liable for damage to wheelchairs, walkers, or other equipment not provided by us, except where our negligence is the proximate cause.

9. Intellectual property

All content on sylhetambulanceservice.com — text, graphics, logo, layout — is the property of Sylhet Ambulance Service and protected by U.S. copyright law. You may not reproduce or republish without written permission.

10. Governing law

These terms are governed by the laws of the State of Texas. Any dispute will be resolved in the courts of Harris County, Texas. If any provision is held unenforceable, the remainder remains in effect.

11. Changes

We may update these terms from time to time. The current version always lives at this URL. Continued use of our services after changes constitutes acceptance.
