These Terms of Service ("Terms") govern your use of the website sylhetambulanceservice.com and any transportation services provided by Sylhet Ambulance Service ("we", "our"). By booking a trip or using our website you agree to these Terms.
We provide non-emergency medical transportation only. In case of medical emergency dial 911 immediately. We do not diagnose, treat, or provide medical care during transport beyond basic passenger-assist and BLS-level first aid.
Bookings can be made by phone at 631-543-5459, by email, or through the request form on our Contact page. All bookings are confirmed only after we send written or verbal confirmation with a pickup window, vehicle type and quoted price.
Written quotes are provided before every booking. We accept credit/debit cards, ACH, HSA/FSA cards, personal checks, and direct insurance billing where covered. Invoices for self-pay trips are due within 30 days of receipt.
Cancellations made at least 4 hours before the scheduled pickup time are free. Late cancellations and no-shows may be billed a $35 dispatch fee. Standing-order cancellations require 24 hours' notice.
Passengers and companions agree to (a) comply with driver instructions relating to safety and vehicle operation; (b) refrain from smoking, alcohol, or illegal substances during transport; (c) remain seated and restrained while the vehicle is in motion; and (d) not photograph or record other patients. We reserve the right to refuse or terminate transport when these rules are violated.
Sylhet Ambulance Service carries $1,000,000 in commercial auto liability and $5,000,000 umbrella coverage. Our liability for any claim arising out of transport shall not exceed the limits of the applicable insurance policy. We are not liable for delays caused by weather, traffic, hospital scheduling changes, or facility-side administrative issues.
You may not (a) reverse-engineer or scrape the site; (b) upload malicious code; (c) impersonate another person; or (d) use the site to violate any law. Content on the site is copyrighted by Sylhet Ambulance Service and may not be reproduced without written permission.
The site may link to third-party resources (Google Maps, insurance carrier portals, hospital websites). We are not responsible for the content, privacy practices, or availability of third-party sites.
These Terms are governed by the laws of the State of New York. Any dispute shall be resolved exclusively in the state or federal courts located in Suffolk County, New York.
We may amend these Terms from time to time. Continued use of our services after posting constitutes acceptance of the revised Terms.
